There was a time—and really not all that long ago—when the orthopedics industry, and in particular, the total joint replacement segment, was defined by its local, small-scale nature. Local suppliers, acting much like design centers and machine shops, turned out implants on behalf of local surgeons with whom they formed close relationships. Even today, the critical importance of independent distribution is, in many ways, a reminder of how important the local and particular have historically been to orthopedics.
All that began to change dramatically two decades ago with the boom in total hip and knee replacements, as large-scale production and mass marketing became increasingly important—a phenomenon not at all rare in business broadly speaking. Moreover, as orthopedics companies found striking success in the total joint market, the natural trajectory of those successful businesses pushed them toward larger-scale production and broader strategic visions. Still, even as recently as a couple of years ago, it wasn't hard to find senior executives in the orthopedics industry dismissing the importance of scale and critical mass in orthopedics, particularly when weighed against things such as close relationships with surgeons and technology advances—this, even as the waves of consolidation and concentration in the industry (and the myriad rumors suggesting other combinations) belied the avowed irrelevance of critical mass.
